Procedure for Enrollment

WELCOME

Southampton Middle School has a registration process before students can be admitted. 

Required Documents:

• Social Security Card of Student
• Birth Certificate of the Student
• Current Immunization  *** 6th grade students must have proof of TDap Shot***
• Individual Education Program (IEP) (if applicable)
• 2 forms of Proof of Residence in Southampton County (Out of district please make an appointment to be seen at the School Board Office)
  • Property tax records which indicates location of homestead
  • Mortgage documents or property deed
  • Apartment or home lease
  • Current utility bills showing residence address
  • Photo Identification of Parent/Guardian
  • Voter precinct identification
  • Automobile registration
  • Affidavit and/or personal visit by a designated School Division official
• Custody documents (if applicable)
